The ASSI membership program arranges for Home Care/Adult Day Care/Companion services in the comfort of your own home! The membership is also designed to contain and reduce the health care cost to members through a network of providers with a Complimentary Discount Medical Plan. There are no health restrictions or paper work. The services provided are of a non-medical nature. All plans are 12 months, hours do not multiply. This Field Issued Contract is retained by the new member at the time of purchase. ASSI may change service providers at its sole discretion. Non payment of fees will result in cancellation of memberships. Please allow 48-72 hours for the commencement of services. Service hours are Monday-Friday 9 AM to 5 PM.

QUALIFICATIONS AND MEMBERSHIPS

Part A. Just call the toll free customer service number and begin to receive services.

Part B. Just call the toll free customer service number. You must be confined to your home and make yourself available to the phone. In order to receive services under Part A or B, your membership must be in good standing for 180 days. Members can mix or match hours or days up to the total amount in each membership.

COMPLIMENTARY DISCOUNT MEDICAL PLAN

Included with all Memberships. The medical discount plan continues without interruption including the long term care/assisted living facility/skilled nursing facility. (This is not insurance)

CANCELLATION

All memberships include a one time 30-day cancellation period. The cancellation notice must be submitted in writing and by a signed letter post marked within the 30-days from the membership’s contract date. (no faxes, no emails)

Full refunds will be made during this time only.

after 30-days, you may cancel your membership however; ASSI is under no obligation to refund any portion of your membership fee. In the event of death, your estate will be refunded on a prorated basis, providing the contract has not been utilized. A copy of the death certificate is required.

RESTORATION

After a member has exhausted all of their Part A and Part B services, the benefits will be restored following 180 days of non-use. Memberships must be paid during the restoration period. The complimentary Discount Medical Plan may be used at all times.
